How is genuine leather (Genuine Leather) made?
You don’t get fake leather when you buy genuine – you just don’t get the top. Actually, this just means it’s not top grain or full grain. It could actually mean many different things. It is deliberately vague.

In many cases, genuine leather is actually split leather. That means the lower (inner) layers of the hide are removed. The outer parts of better quality are "top grain". The ability to take multiple layers from one hide depends on the thickness of the hide. Some skins, like cows, are often able to be split into different levels. Genuine leather can come from the middle layers – between top grain and suede. That is in terms of position in the hide – not in terms of quality.

Sometimes genuine leather can also be mixed with leather scraps and artificial materials and binders.

Quality
Genuine leather is at least leather. Of the various possible grains and cuts, it is at the bottom of the list. Compared to high-quality leather such as full grain and top grain, it is certainly an inferior product.
